MHHC delivers a comprehensive continuum of preventive, primary, specialty,
behavioral health, and social services designed to meet the full range
of patient needs across the lifespan. For especially vulnerable populations,
we offer an integrated care model that ensures patients receive whole-person
services within a community-based network.
Age Well
A certified Age-Friendly Health program, serving 4,000 older adults annually
with wellness coaching, geriatrics, dental, behavioral health, PT/OT,
chronic disease management, and nutrition support.
Maternal Health & Hope Coalition
Addresses the large disparity in pregnancy health outcomes among women
of color through community partnerships and connections to care and resources.
Activities include prenatal yoga, safe sleep education, breastfeeding
support, maternal health literacy, community resource fairs, and midwife/doula services.
Young Adult and Adolescent Health Unit (YAAHU)
Delivers trauma-informed, developmentally appropriate care for ages 11–26,
including compassionate & informed care, reproductive health, STI/HIV
prevention, and behavioral health.
Eat Well
Reduces the risk of chronic conditions such as obesity, diabetes, and hypertension
by providing fresh food access and nutrition education through a farmers’
market, food pantries, cooking demonstrations, Health Bucks, and older
adult food programming.
Healthy Destination: Food to Medicine
Provides a gateway to the health care system by providing medically tailored
food boxes after an initial primary care visit at our flagship facility.
The program targets the nearly 20% of residents in our under-resourced
and impoverished neighborhood who have no usual source of care by incentivizing
them to engage with the healthcare system.
Read Well
Engages more than 1,700 children annually with bilingual book distribution,
storytelling events, and early childhood development supports such as
Reach Out and Read, Prescription for Play, and HealthySteps.
LiveWell – School-Based Wellness Services
Provides preventive care, screenings, immunizations, nutrition counseling,
mental health support, and health education directly in MHHC’s school-based
health centers, improving access to care, school attendance, and academic
readiness.
