Clinical Informatics Associate & Programmer (Full Time)
Clinical Informatics
The Clinical Informatics Associate is responsible for assisting with the implementation, maintenance and enhancing usability of the electronic health record (EHR) systems. Responsible for building clinical templates using VFE programming based on clinical end-users’s feedback and analysis of existing data. Provides high-level EHR user support. Must be detail-oriented, organized, self-motivated, able to work independently and in groups, meet project deadlines, and strong problem solving skills.

Clinical Informatics Populations Health Report Writer(Full Time)

The CI Population Health Reports Writer is responsible for promoting individual and population health by identifying, monitoring, and providing data for clinical staff to outreach to patients with the goal of improving quality of care. Charge Registered Nurse(Full Time)
Pediatrics & Women’s Health
The Charge Nurse is responsible for the daily operation of the clinical unit, including scheduling of nursing and support staff, managing patient flow, maintaining supplies and equipment necessary for the care of the patient. The Charge Nurse conducts staff meetings and works in collaboration with other managers and departments. The Charge Nurse is responsible for ensuring all nursing staff of the unit is current in competencies, licensures, certifications, and other annual training. The Charge Nurse collects data for nursing and organization performance improvement activities and serves as an active member of committees and task forces.
Qualifications: Graduate of an accredited school of nursing. Preferably with a baccalaureate degree in nursing or health administration; Minimum two (2) years clinical nursing experience, preferably in an ambulatory care setting; NYS license as a registered professional nurse; Basic Life Support (BLS) certification required. At least one year for Supervisory experience.
Community Health Worker(Part Time)
Community Health Worker Program
Under the supervision of the Program Coordinator, the Community Health Worker (CHW), identifies and assists individuals and families to access needed preventive and primary health care services such as family planning, intervention, WIC and other nutrition services. The CHW conducts health education to increase family awareness, conducts basic health assessments, and assists families with health behavior changes. The CHW serves as a liaison between community agencies and families sensitizing providers to the needs of minority populations served by the Community Health Worker Program. The CHW maintains contact with individuals and families through at least monthly home visits.
Qualifications: High School Diploma or GED required; Demonstrate evidence of strong interpersonal, communication, and writing skills; Must be able to work in an unstructured (field) setting and be at ease in meeting and talking to people in various situations; Ability to work flexible hours, including some evening hours; Bilingual (English/Spanish) required

The Development Associate is a key member of the Development Team who:
- Provides administrative and analytical support to ensure the department raises at least $1M from fundraising activities annually

Family Support Worker(Part Time & Full Time)
Healthy Families
Healthy Families is a home visitation program for pregnant women and families with newborns. This program seeks to prevent child abuse, promote positive parenting and link the family to a medical provider and community resources.
The family support worker is responsible for initiating and maintaining regular (at least weekly) and long term (up to five years) contact/support with families. This activity will occur primarily within the family’s home; each visit will last for at least one hour. The intervention should be family centered and strength based to empower the family to meet their needs.
Qualifications: High School Diploma required or equivalency (GED) with some experience with working with families or in the human service field; Bilingual English/Spanish is highly preferred
Health Educator / Outreach Worker (Full Time)

Provide education and information on reproductive health topics such as reproduction, contraceptive methods, family planning, pregnancy, relationships, HIV, STDs, etc. ;Conduct group and individual sessions on reproductive health topics; Promote Family Planning-focused health discussions at various MHHC sites, schools, industry, and other community agencies; Research, select and /or prepare and disseminate educational and informational materials relevant to health promotion and disease prevention with focus on Family Planning, Women’s Health and Infant Mortality reduction InitiativePlans, researches, prepares and implements health educational programs for adolescents, teenagers, parents and teachers in the High school.

LPN (3) (Full Time)

Assists the RN in the implementation of the nursing process on his/her assigned patients. Tasks include case finding, health teaching, health counseling and provisions of supportive and restorative care. Along with these clinical duties, the LPN at Morris Heights Health Center assumes administrative duties necessary for the optimal running of the clinical units

Medical Case Manager(Full Time)

Under the supervision of the Clinical Director and HIV Medical Director, provides comprehensive case management to HIV positive patients. Will function as an advocate & referral source and is responsible for the coordination of medical care and psycho social services for the patient, ensuring compliance with medical appointments, providing medication adherence support and overall holistic care and monitoring of patients

A licensed practitioner who under the guidance of the Director of Pediatrics, Adolescents and School Health Services coordinates and implements the School Health Program. Provides comprehensive healthcare and referral services to students and parents/guardians.
Qualifications: Graduated from an approved Nurse Practitioner Training Program; Two or more years of experience as a Nurse Practitioner is preferred; Certified by the American Nurses Credentialing Center (ANCC).

Program Associate(Full Time)

The Healthy Families Program provides home-based services to expectant families and new parents, beginning prenatally or shortly after the birth of a child. The program services are geared towards preventing child abuse and neglect. The goals of the program include: a) Supporting positive parent-child bonding and relationships; b) Promoting optimal child health and development; c) Enhancing parental self-sufficiency. The Program Supervisor provides direct clinical supervision to five FSW/FAWs, ensures quality service delivery and compliance with policies and procedures governing the Healthy Families Program.
Qualifications: An advanced degree in Health or Human Service Field, or a bachelor’s degree in a Health or Human Service field; Five years experience in a home visiting program, with clinical supervisory experience preferred; Solid experience with multi-challenged families, including work in the field of child abuse or family violence and previous supervisory experience in motivating and supporting staff in a stressful work environment.
Program Director(Full Time)
School Based Health Center
This position is accountable for direct oversight of the School Based Health Center Network Program, a program offering onsite medical, social work, health education, dental and youth development programming to various schools in NYC. This position will be required to oversee operations at the ever-growing SBHC network and requires hands on practice in ensuring that everything that is needed for daily operations is available, (supplies, staffing, and equipment). This position will be engaged in the Performance Improvement Committee for SBHC and will be required to identify and in some cases lead collaborative and non-invasive research.

Program Supervisor(Full Time)
The Healthy Families Program provides home-based services to expectant families and new parents, beginning prenatally or shortly after the birth of a child. Families are screened to identify risk factors and stressors. The program services are geared towards preventing child abuse and neglect. Program Supervisor provides case supervision, education, and direct observation or provision of service delivery to achieve program objectives. In this capacity, the supervisor will assist on all aspects of program planning, events, performance and implementation to ensure compliance with contract. In, addition, the Program Supervisor will be responsible for the FAW role by screening, conducting assessments, marketing program through outreach.
Qualifications: An advanced degree in Health or Human Service Field, or a bachelor’s degree in a Health or Human Service field and five years experience in a home visiting program, with clinical supervisory experience preferred. Site Director (Full Time)

Under the supervision of the Chief Operating Officer, the Site Director will administer the daily operations of an outpatient facility including overall coordination of all patient care activities. The Director will work with the Chief Medical Director and clinical leaders of each unit to coordinate and develop programs, arrange staffing, and oversee operations that meet federal, state, and local regulatory requirements. This individual should have 3 to 5 years experience working within a hospital ambulatory system
Qualifications: Previous Supervisory experience is required; The individual in this position should have 3 to 5 years in managing and analyzing medical group operations within a community health center, a hospital system or other ambulatory system; The individual should possess a Bachelors degree from an accredited university or college; Bilingual English/Spanish is highly preferred
Senior Planning Director (Full Time)

Responsible for the day to day administration of the organization’s grant development and grants management processes. The incumbent will conduct short and long range planning processes, culminating in the production of the organization’s Strategic Plan.
Qualifications: Master’s degree (M.A.) in Health Administration or a related field with at least three (3) solid years of experience in program development and business strategic planning. This position requires an in-depth, thorough and strategic thinker who can respond to changes in the business & healthcare landscape in order to capitalize on opportunities that will allow MHHC to expand and meet strategic goals. Candidate should also have strong knowledge of primary health care programs as well as budget development and cost analysis.
